US-based Teammates are eligible for health benefits, which are determined jointly by the People and Finance teams on an annual basis. Our benefit plans and information can all be found Sequoia.

Eligibility

All US-based full-time employees and their dependents are eligible for health benefits through Sourcegraph. Benefits are effective on your date of hire.

Others eligible for benefits include:

  • Your legal spouse or domestic partner,
  • Your unmarried dependent children up until age 26 (including legally adopted and stepchildren), and/or
  • Any dependent child who reaches the limiting age and is incapable of self-support because of a mental or physical disability.

Medical / Dental / Vision Benefits

  • We offer high-quality medical, dental, and vision care for all US-based full-time employees and their dependents. Sourcegraph covers 100% of premiums for medical, dental, and vision insurance for employees and dependents (for US-based full-time employees).

Life Insurance Benefits

  • We offer basic life insurance and cover the full premium for all US-based full-time employees.

Enrollment

This is your once-a-year chance to enroll in or make changes to your employer-sponsored health plan. You must enroll either upon joining Sourcegraph and/or during Sourcegraph’s annual open enrollment period.

During enrollment

  • You must elect or decline benefits
  • You may add or drop dependents
  • You may set contributions to tax-advantaged accounts

You cannot make changes outside of open enrollment unless you have a qualifying life event (QLE).

New hire benefits enrollment:

All full-time, US-based Teammates are eligible for benefits. Insurance benefits for US-based Teammates become active on your start date.

US-based new hires have 30 days from their start date to enroll in insurance coverage through Rippling. You will receive a Rippling invite at your personal email by your first week. You will use Rippling to find your pay stubs and tax forms, set up your payment information and employment eligibility (I-9), and select your health insurance plans. If you do not have an invite to Rippling by your first week, please contact the People Team in #ask-people-team in Slack or people-ops@sourcegraph.com.

You can waive your insurance coverage through Rippling. Insurance coverage is automatically waived for any teammate who doesn’t select insurance during their first 30 days.

Changes to personal information

Updates to the following information must be noted in both Bamboo and Rippling: An employee’s name, address, phone number, gender, Social Security Number, or date of birth, as well as updates to a dependent’s name, relationship type, gender, Social Security Number, or date of birth.

2024 Open enrollment:

What: Open Enrollment is your once-a-year opportunity to review and change your plans, add/or remove dependents, and review your costs and contribution for benefits in the coming year. Outside of open enrollment, you can only make changes to your insurance if you have a “qualified life event” - such as a marriage, or the birth of a child. You can find a recording of our Open Enrollment Session here.

  • Note: If you start at Sourcegraph during open enrollment Rippling will walk you through new hire enrollment for 2023 benefits and open enrollment for 2024 benefits. If you start after Rippling will also walk you through both new hire enrollment and a special open enrollment window.

How: To make changes to current benefit elections, or confirm existing elections, please follow instructions in your Rippling account (use the taskbar on the left-hand side to navigate to the heart icon labeled “insurance and benefits.” From here, you should see the option to start your plan selections). Any plan changes, or confirmations, will become effective as of .

  • Our 401k Plan is administered by Human Interest. You can view or change your elections via the Human Interest App, accessed through your Rippling homepage.

Benefits Support:

We use Sequoia Consulting Group as our benefits & perks broker. They work with our People Team and carriers to provide the best possible benefits to our teammates. Sequoia’s advocate team works directly with Teammates and/or their dependents to answer questions, provide guidance, and work with the carriers on the teammates behalf when possible.

A few examples of what to reach out to the advocate team include:

Digital ID Cards:

All digital insurance cards are located in Sequoia, which is our benefits broker. All health insurance cards and coverage questions are handled by Sequoia. You will receive physical medical cards, but dental will only be available through the Sequoia app. Vision cards are not provided, but your insurance can be verified with your vision provider with your name, SSN, and stating your plan is with Equitable Vision.

US-based Teammates can access virtual insurance member IDs in the Sequoia mobile app within 7-10 business days after you complete enrollment. You will receive your physical insurance member ID cards via mail 2-4 weeks after you complete enrollment.

Q. Does Sourcegraph offer short term disability and long term disability?

  • At this time, Sourcegraph does not offer STD and LTD but we are always evaluating our benefit options. Please review our Medical Leave of Absence Policy.

Q. Do our health benefits cover us while traveling internationally?

  • They do not. However, we do have separate insurance covering if you are traveling internationally for work (sourcegraph sponsored travel). Please see the details below.
    • Traveling for work: If you are traveling on behalf of Sourcegraph for work, we have a corporate policy to provide insurance to all teammates (not just US-based). The insurance covers any travel you are doing for Sourcegraph in a business capacity and includes: medical assistance and travel assistance (lost baggage, travel alerts, etc.). Please review the information here for complete details.
    • Traveling for fun/vacation: We recommend using a travel insurance for the duration of your trip and expensing this. A common one used for US teammates is GeoBlue.

Q. How do I make changes to my insurance in the case of a qualifying life event (QLE)?

US teammates:

  • A Qualifying Life Event (QLE) is a change in your situation, such as getting married or having a baby, that can make you eligible to enroll in health insurance outside of Sourcegraph’s annual Open Enrollment period. If you have experienced a Qualifying Life Event (QLE) and need to update your insurance, you will initiate this process via Rippling. Note: the People Team cannot make updates to benefits on behalf of teammates outside of new hire and open enrollment periods.

Q. What do I do if I have a gap in insurance coverage? (US teammates)

  • If you are joining Sourcegraph, meaning you’ve accepted an offer with Sourcegraph, and have a gap between your start date and the end the benefit coverage period w/prior employer. Please contact your prior employer regarding their COBRA benefits.
  • If you are leaving Sourcegraph, benefits will expire at the end of the month that you leave Sourcegraph. If you want to avoid having a gap in coverage, you can explore COBRA options. If you have dental and vision insurance at the time you leave Sourcegraph, you will continue receiving those same benefits through COBRA.e benefits through COBRA. Benefits through COBRA will expire at the end of the month that you leave Sourcegraph.

Q. What do I do if my medication or procedure is not covered by our insurance plan?

US teammates:

  • Our insurance plan cannot be augmented to cover specific medications. Please work with your healthcare provider and contact Sequoia’s advocate team to verify.

Non-US teammates:

  • For Vision, we do not have physical or virtual cards. Your provider can verify insurance with your name, SSN, and stating your vision insurance is Equitable.

Q. Does Sourcegraph match my 401k?

  • Not at this time.

Q. Who handles our 401k?

  • Human Interest for US based teammates. You will receive an invite shortly after enrolling in Rippling. You can find instructions to enroll inHuman Interest here.

Q: When can I enroll in my 401k plan?

US teammates:

  • You are eligible to enroll immediately upon hire and can make changes or updates at any point. You will enroll in Human Interest (after receiving your invite) or via Rippling (if you see a Human Interest single-sign-in option on your dashboard).

Notices (Sent Oct 2023)

HIPAA Notice of Special Enrollment Rights

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) provides employees additional opportunities to enroll in a group health plan if they experience a loss of other coverage or certain life events. This Notice will provide additional information about your Special Enrollment Rights, and it is also located in the Plan’s wrap Summary Plan Description document.

HIPAA Notice of Privacy Practices

The HIPAA Privacy Rule gives individuals a fundamental right to be informed of the privacy practices of their health plans and of most of their health care providers, as well as to be informed of their privacy rights with respect to their personal health information. Health plans and covered health care providers are required to develop and distribute a notice that provides a clear explanation of these rights and practices. The Privacy Rule provides that an individual has a right to adequate notice of how a covered entity may use and disclose protected health information about the individual, as well as his or her rights, and the covered entity’s obligations with respect to that information. This notice is also located in the Plan’s wrap Summary Plan Description document.

Women’s Health and Cancer Rights Act

The Women’s Health and Cancer Rights Act (WHCRA) provides that group health plans and health insurance issuers that provide coverage for medical and surgical benefits with respect to mastectomies must also cover certain post-mastectomy benefits, including reconstructive surgery and the treatment of complications (such as lymphedema). The Notice will provide additional information and is also located in the Plan’s wrap Summary Plan Description document.

Newborn’s and Mothers’ Health Protection Act of 1996

The Newborns’ Act was signed into law on September 26, 1996 and includes important protections for mothers and their newborn children regarding the length of the hospital stay following childbirth. The Notice will provide additional information and is also located in the Plan’s wrap Summary Plan Description document.

Children’s Health Insurance Program (CHIP) Notice

The model CHIP notice lists the states in which CHIP premium assistance is available.

Medicare Part D Notice

The Medicare Prescription Drug Improvement and Modernization Act of 2003 requires group health plans that include prescription drug coverage to disclose annually to individuals eligible for Part D whether the plan’s coverage is “creditable,” meaning that the group health plan coverage is at a minimum equivalent to Part D coverage. Please note that Sourcegraph’s plans are creditable.

Individuals who do not enroll in Part D coverage when first eligible, and those that go 63 days or longer without creditable coverage, will have to pay higher premiums if they subsequently enroll in a Part D plan. You will need this notice to confirm your status when enrolling for Medicare Part D. Please note that you are responsible for providing this notice to your eligible dependents.
